Disabled Person's Placard

To obtain a Disabled Person's Placard (which hangs from the rear-view mirror), an MV9-D (Disabled Person's Parking Affidavit) must be completed by a licensed practitioner of the healing arts and must be notarized. Bring the completed form along with your Georgia driver's license or Georgia ID card to a Gwinnett County tag office.

 

Your physician must designate on form MV9-D whether the disability is temporary or permanent. Temporary permits are issued for six months. To renew a temporary placard, a newly completed form MV9-D must be submitted.

 

Permanent placards must be replaced every four years but do not require the submittal of a new form. For license plates, the disability must be permanent.
