Affidavit of Correction (T-11)

The T-11 is used to explain allowable corrections in an assignment section on the back of the title or statement of origin. It must be completed by the seller and the seller's signature must be notarized. Examples of when to use the form would be if the seller signs in the incorrect space or a buyer decided not to continue with the transaction and the seller is going to sell it to someone else. One thin line can be drawn through the incorrect information. It cannot be used for titles where white out, correction tape or complete scratching out of the information has occurred.

Application for Replacement Plate and/or Decal (MV-7)

This form is used for reissuing a plate and/or decal if it is stolen, lost, damaged, or lost in the mail and will be completed when you come into the Tag Office. A copy is then sent to the police to keep on record. Whenever form MV-7 is required, a Tag Office associate will complete it for you. If the plate and/or decal has been lost in the mail, a Tag Office associate will notarize it as well.

Bill of Sale (T-7)

This form can be used to transfer ownership of personal property. It can be used in lieu of a title on motor vehicles 1985 and older where there is no title and trailers that are not titled in Georgia (weighing less than 2000 lbs).

Extension of Dealer Tag (T-226)

This form is used when requesting an extension because a dealer is still processing the title paperwork on the buyer’s behalf. The form can be completed and brought into the Tag Offices where the buyer resides along with the dealer drive out tag, bill of sale and proof of insurance. An extension can be obtained for up to 30 additional days if the buyer comes in within five days of the expiration date on the dealer drive-out tag.

Lien Release (T-4)

When applying for a duplicate title and there is a security interest still shown on the vehicle record, this form can be used to release that interest when completed by the financial institution. All signatures must be original, no faxes or email.

Out of Area Extension Application

If you (or a family member) and your Georgia-registered vehicle are temporarily located well outside the 13-county emission testing area of Georgia due to school, military, or business obligations you may qualify for an Out of Area Extension. Application for an Out of Area Extension must be submitted prior to the vehicle registration renewal date.

Prestige Application (MV-9B)

This form is completed when requesting the manufacturing of a Prestige License Plate for either a motor vehicle or motorcycle. There is a designated area on the form to list 1st, 2nd, 3rd choices for the plate.

If the registered owner of a motor vehicle is 65 years or older and the vehicle they are renewing/registering is at least 10 years old and is driven less than 5,000 miles per year, this form can be completed to exempt the emissions for that motor vehicle. The vehicle must have a functional odometer and a new form will need to be completed each year with the current mileage on the vehicle.

Title Bond Paperwork

A title bond can be used when a title cannot be located or corrected on vehicles 1986 and newer. Title Bond Companies are used to secure a title. The forms required for the title bond company to complete are: MV-46, MV-46A, T-22B (vehicle inspection by a law enforcement officer), T-4 (lien release by a security interest), T-7 (Bill of Sale) and MV-1 (Title Application)
